  • Equity Scholarship Application Form (if applicable)
  • Documents to verify your situation i.e. medical certificates, letters verifying circumstances, etc.
  • Invitation to Apply
  • Summary describing your proposed thesis (at least 3 pages in length) including methodology and bibliography
  • Evidence of scholarships/sponsorships awarded for this research program by an external body (e.g. employer or home government)
  • On-site Supervisor form (for external students, where applicable)
  • English Proficiency Test
  • Certified copies of official academic transcripts (listing all units attempted, marks and grades achieved) for all completed qualifications
  • Certified copies of your degree certificates/testamurs
  • An explanation of/link to grading schema/s
  • Evidence of any previously awarded awards, prizes and/or scholarships
  • Written evidence of research or work experience, such as a letter from your employer confirming your employment and duties performed
  • Contact details of two referees
  • Copies of any referee letters or reports
  • Evidence of any published research (e.g. conference, peer reviewed journal article or book/book chapter). This may include proof of acceptance, conference abstracts or the front pages of the publication
  • Any additional documentation required to support your application
  • Academic Curriculum Vitae
  • Agent Nomination Form (if an international student applying with help from an approved Monash agent )
